The Trust Fund is making changes to the vendors who provide your podiatry, mental health and substance use treatment benefits. Beginning January 1, 2025, Blue Shield of California will replace both the Podiatry Plan Inc. for podiatry benefits and Uprise Health for mental health and substance use treatment benefits.
What This Change Means for You
Blue Shield PPO Members will now have access to the entire network of Blue Shield In-Network Providers for podiatry and mental health and substance use treatment benefits.
Who gets an ID?
The Trust Fund Office is preparing to generate updated 2025 ID cards online for Active and Non-Medicare Retiree members in the Blue Shield PPO Plan.
When will 2025 Cards be available online?
The new 2025 ID card will be available to view and print from your ufcwtrust.com Participant Account in Mid-January.
