2023 Sick Leave Payout Checks
Sick Leave Payout checks for 2023 will be mailed by March 8, 2024. Please allow 5-10 business days for this mailing to arrive.
Members who did not complete a 2023 Annual Verification will have their Sick Leave payout held.
You can access the Annual Verification form by clicking the red button below:
To complete and submit 2023 Annual Verification:
1. Print the Form and complete it
2. Write “2023 AV” at the top. Remember to sign and date the Form.
3. You can submit the Form one of the following ways:
Online:
– CLICK HERE to log in to your ufcwtrust.com Participant Account and upload the form.
– Select the Correspondence tab then the “Upload” button under My Submitted Documents.
*Be sure to select “All Enrollment docs (Active)” from the drop down menu

If you want to receive your payout quickly and have not submitted Annual Verification for 2023, please submit your form by April 1, 2024 to ensure it’s included in the next check disbursement.
Please allow seven (7) to ten (10) business days for your Annual Verification to be processed. Your Annual Sick Leave Payout will be mailed to your home address after your Annual Verification is processed.
